A referral from a physician, physician’s assistant or nurse practioner is required for SureSteps, Inc. to bill Medicare or other insurance providers for services. The referral may come from a primary care physician’s office or the office of a specialist, such as a neurologist, rheumatologist or orthopedic surgeon.

The order should be faxed to us on our fax line:  919-838-0439. The order may be written on the SureSteps order form or on a generic prescription script. The faxed referral should include a patient demographics page so we will know how to contact the patient.
